Here's what to bring to your appointment
Save time in the office by completing some forms before your appointment. This includes the patient registration and medical history forms. Also, the medical release form gives your doctor permission to send us your health records.
Medicines you’re currently taking
Bring your medicines to your appointment. This includes any vitamins or supplements that you take. It’s best to bring them in their original bottles. If you don’t have the original bottles, bring a list of your medicines and the amounts you take.
Immunization (shots) record
Bring a record of any shots you’ve had, if you have it. This record is especially helpful if we are seeing your child or adolescent.
Advance care planning documents
Please bring any advance care planning documents with you. This includes your advance care directive if you have one. Your doctor will go over it with you during your appointment.
Medical insurance card and photo ID
Always bring your medical insurance card and photo ID to your appointment.
