What is a HouseCalls visit?
Your health is important to us. So we want to make it as simple as possible for you to get access to care you may need. Through our HouseCalls program, we offer you an annual health visit right in the comfort and privacy of your own home.
We’ll send a licensed clinician to your home. This might be a nurse practitioner, a doctor or a physician assistant. You can pick the date and time that works best for you. It gives you a chance to discuss your health care needs and create a plan for prevention.
Who can have a HouseCalls visit?
Eligibility depends on the state in which you live and which plan you have. Check with your health plan to see if HouseCalls is an option for you.
What happens during a HouseCalls visit?
During a HouseCalls visit, a licensed clinician will:
- Go over your medical history.
- Discuss any health concerns you may have.
- Perform a wellness check-up and health screenings.
The clinician will also:
- Review your medications.
- Arrange for you to have certain lab tests if needed.
- Assess your living conditions to see how they may affect your health.
- Provide information about your health condition(s).
The clinician will send information about the visit to your doctor. You may also be referred to specialized care programs if they are part of your plan. You can have a HouseCalls visit once per year. It is not meant to replace your regular visits to your doctor.
How do I set up a HouseCalls visit?
If you are eligible for a HouseCalls visit, you’ll get a call about 6–8 weeks after your health plan is effective. They’ll check to see if you want to set up a home visit. You can also call your health plan to see if you are eligible and learn more about the program.
