Start with your health insurance plan
OptumCare® accepts new patients with many different health insurance plans. We accept most HMO, PPO, POS, Original Medicare, Medicare Supplement and Medicare Advantage plans.
HMO or Medicare Advantage Plans
Do you have an HMO (health maintenance organization) or Medicare Advantage (MA) plan? Or are you signing up for a new HMO or MA plan? The first step is to understand how your health plan, your medical network and doctor's office work together.
To join OptumCare, you’ll need to choose a plan that gives you access to one of our networks. Then you’ll need to choose your primary care provider. We'll walk you through the steps you need to take.
PPO plans or Original Medicare
In Phoenix, our OptumCare Primary Care medical offices accept many PPO (preferred provider organization) plans and Original Medicare. In most cases you can simply call the doctor’s office of your choice and make an appointment. Go to our Locations tab to find a doctor’s office near you that is part of OptumCare.

- Find out if the doctor is "in network."
The next step is to check with the doctor’s office to see if they accept your health plan. The medical offices that are a part of OptumCare accept most major health plans. However, it may vary by location.
Before you make an appointment, ask if the doctor is “in network” for your health plan. If the doctor is “out of network,” your costs may be higher. You can call your health plan to ask if the doctor is in your network.
