Health maintenance organizations (HMOs)
When you have an HMO insurance plan, you choose a medical network and a primary care doctor who oversees your care. See HMO plans we accept.
If you have an HMO plan, you pick a medical network and one primary care doctor in that network to oversee your care. Your doctor works with the hospitals, specialists and other care providers in your network so you get the care you need, when you need it.
An HMO plan usually won't pay for visits to providers outside of your network. Check with your insurance plan to find out what visits they cover.
To become a new HMO patient with Optum:

- See our list of accepted medical networks. If you’re not sure which network you’re in, contact your health insurance company. Each network includes certain doctors and hospitals.
- If your chosen clinic accepts your health insurance plan, the next step is to choose your primary care doctor. Search our locations and doctors Each doctor's page will tell you what network(s) they are in and their network ID numbers.
- Tell your health insurance company that you want to switch to a new primary care doctor. Give them the doctor's name, the network you want and the doctor's network ID number. They will tell you what date you can start going to your new doctor.
