  • Optum is proud to be part of ACOs, which are groups of doctors and other health care providers that work together to make sure you get the care you need, when you need it.

    The goal of an ACO is to organize your care and communication about your care, so you get the highest quality care possible.

  • ACOs help coordinate your health care. Coordinated care means that everyone on your health care team works together to make sure you get the right care at the right time. 

    Everyone on your care team talks to each other. The goal is to stop medical errors and needless repetition of services.

  • The Anthem Enhanced Personal Health Care program has a care team to help meet the needs of select Anthem PPO health plan patients with certain medical problems.

  • Cigna’s Collaborative Care program allows select Cigna PPO health plan patients to get special coordinated care services offered by Optum.
