How hospice care works
Our team starts by having a conversation with you to understand your wishes. We then help you create an advance care plan. We make sure you have a clear plan of care so that your family and care providers know your wishes, now and in the future.

This team of specialists meet weekly to make sure your care plan provides the right care for you:
- Hospice doctors
- Certified nursing assistants and home health aides
- Social workers
- Bereavement coordinators
- Trained volunteers
Four levels of hospice care
- Routine home care: Care in your own home.
- Respite care: To give your caregiver a period of rest.
- Continuous care: Around-the-clock care for a short time during a crisis.
- General inpatient care: Care for a brief time if you need to go to the hospital.
The right time for hospice care
Hospice care usually begins during the final stages of a serious illness. Most patients start hospice care when their doctor believes they have about six months left to live. Talk to your family and doctor to see if hospice care is right for you.
How will hospice care work with my current treatments?Text
You usually need to decide to stop treatment before you can get hospice care. But you can still get palliative care to help manage your pain and symptoms. Talk to your doctor or hospice provider about your options for treatments provided as part of hospice care.CLOSE

How do I get hospice care?Text
Two doctors need to certify that you are eligible to get hospice care. Usually this is one of the doctors who is caring for you and an OptumCare Hospice Care doctor. Our team members will help you start the process and talk with you about what to expect.CLOSE
Will insurance cover my hospice care?Text
Most insurance plans, including Medicare and Medicaid, cover hospice care. This includes prescriptions and medical equipment. An OptumCare team member can help you figure out what your health plan will cover.CLOSE
