The Health Care Center and OptumCare

  • The Los Alamitos Medical Center decided not to renew its lease after 30 years. The Golden Rain Foundation Board of Directors thoroughly reviewed proposals, and chose OptumCare. OptumCare will take over the HCC lease for the next five years. There is an option for a five-year extension.
  • OptumCare is reinventing health care to help keep people healthier and feeling their best. We're led by physicians who understand the needs of patients and providers. Our goal is to make the health care system work better for you.
  • No. OptumCare provides direct care to patients. We are the doctors’ offices.

    Monarch HealthCare is a health care services network. Along with your health insurance, Monarch provides you access to many hospitals, urgent care centers, and labs when you need them. If you go to an OptumCare office, you are in the Monarch network.

    Your insurance plan handles your benefits and copays. Monarch works with your insurance to take care of authorizations, referrals, billing, and claims.

Doctors, specialists and services

  • Our goal is to have patients see their doctors when needed. Here’s how:

    • Chris Nguyen, MD and Al Pita, MD will keep seeing patients in the Seal Beach Village office.
    • Ellen Dayon, MD, Rudy Haider, MD and Nancy Smith, MD will be at the HCC.
    • Hyunmie (Amy) Kim, geriatric nurse practitioner, will spend time at both.

    We believe this will help meet the needs of both locations during the transition. 

  • Many of the specialists have remained with the HCC. There are also more specialists outside Leisure World you can see.

    To find a specialist, give Monarch HealthCare a call. Monarch is a company that partners with us to give you access to specialists and hospitals. When you call, let Monarch know you are a patient with the Leisure World Health Care Center.

  • We know how important these providers and services are in this community. We plan for them to stay here at the HCC.
  • Yes. The 24-hour on-call nurse will still be available.
  • We’re developing health care programs and looking to add more specialists. We believe these added services will help keep everyone at Leisure World active and healthy.

    The Leisure World Board of Directors will reach out to you soon for feedback. Your thoughts and ideas will help us shape programs that make sense for you.

Getting care

  • Yes. OptumCare and Los Alamitos Hospital have a close relationship. Residents can go to Los Alamitos for care.
  • Yes. We know shuttle service is important to you, and it will keep running. 


  • You’ll need an HMO or Medicare Advantage plan that OptumCare accepts. If it does, tell your plan which OptumCare doctor you want as your primary care doctor. You can find the plan's customer service phone number on the back of your health plan card.

    Not sure which doctor is right for you? You can find printed material about each doctor in the HCC lobby.

  • You can change this by calling your health plan. The customer service line is on the back of your insurance card.

    Your plan will need to know:

    • The name of the doctor you want
    • The location of the doctor’s office (the Health Care Center)

    It’s important to tell the plan that the doctor is part of the Monarch network. This will help make sure you get the right doctor.

    Note: the date you call determines when you can start seeing your new doctor.

    If you call:

    • Before the 15th of the month, you can start seeing your doctor at the start of the next month. For example: If you call on July 10, you can see the new doctor August 1.
    • After the 15th, you’ll need to wait until the month after the next month to start seeing your doctor. For example: If you call on July 18, you can see the new doctor on September 1.

  • OptumCare doctors accept many HMO (health maintenance organization) and PPO (preferred provider organization) plans through the Monarch HealthCare network. 

    To find out if you are part of Monarch, call your plan’s member services line. You’ll find it on the back of your insurance ID card. You can also stop by the HCC and someone will help you. We also accept Medicare and cash payments.
  • OptumCare doctors accept many major health plans through the Monarch network: 

    • Aetna®
    • Alignment Health Plan
    • Anthem Blue Cross®
    • Health Net®
    • Humana®
    • OneCare (HMO special needs plan)
    • OneCare Connect Cal MediConnect
    • SCAN Health Plan®
    • UnitedHealthcare®
  • If you want to get care at the HCC and we don’t take your Medicare Advantage plan, you may want to speak to a licensed insurance agent. This is a professional who can compare different plans to find one that meets your needs.

    Licensed insurance agents host meetings at the HCC from September through December. You can find the meeting dates by visiting the HCC. Or, call the HCC toll-free at 1-833-846-7886, Monday through Friday, 8 a.m. to 5 p.m.
